Appointments
To schedule an appointment please contact our office at 513-891-5860. As a courtesy to our patients we confirm all appointments in advance.
Cancellations
If it is necessary for you to change or cancel an appointment, we request 48 hours advance notice. This will allow us time to work in another patient who may be in need of care. There may be a fee for broken appointments or late cancellations.

THEN:
~ Go into right lane which will bring you up to a right turn onto Montgomery Road
~ Immediately get into left lane.
~ Go 20 yards and turn left at traffic light (9200 Montgomery Station).
~ Go down the driveway to the first stop sign. Make a right into our lot.
~ Look for large "A" on our building - Suite 1A.

Payment / Insurance
We always strive to make the most of your Insurance coverage.
Dental Treatments - Dental claims are electronically filed for you. Co-payments are required at the time of service. Other financial arrangements may be setup prior to treatment. We are a provider for Dental Care Plus and Superior Dental Case Plans.
CareCredit® for dental procedures - We're pleased to offer our patients the CareCredit® card, North America's leading patient payment program. CareCredit works just like a regular credit card, but without high interest rates or other costly penalties. You'll get low monthly payments for healthcare procedures not commonly covered by insurance, including dental procedures. Plus, you can use your card over and over for follow-up appointments.

Medical Therapies for Pain / Sleep Disorders - We are a fee-for-service practice and request payment at the time services are rendered. We are out-of-network for medical insurance plans, but our staff will thoroughly review and coordinate with insurance issues and support insurance claims filings.
There is a 5% discount for cash or payments made by check. We also accept Visa, MasterCard, and American Express.
Our office also offers outside financing. Talk to our staff to see if you are eligible for 3 or 6 months same as cash and extended financing options.
